Specialized OCD treatment to help you break free from the cycle of obsessions and compulsions
If intrusive thoughts take up your mental space and rituals take up your time, you are not alone, and you are not “crazy.” OCD is a treatable condition. At Flourish Counseling and Wellness, our ERP therapists in Asheville, NC help adults and teens loosen OCD’s grip and get back to the things that matter to them.
What Is Exposure and Response Prevention (ERP)?
Exposure and Response Prevention, or ERP, is a form of cognitive behavioral therapy (CBT) and is widely considered the gold-standard psychotherapy for obsessive-compulsive disorder (OCD). It is recommended as a first-line treatment by leading organizations, including the International OCD Foundation and the American Psychiatric Association.
OCD works as a cycle. An unwanted thought, image, or urge (the obsession) triggers intense anxiety or distress. To get relief, you do something to neutralize it, such as checking, washing, counting, seeking reassurance, or replaying the thought in your head (the compulsion). The relief is real but short-lived, and it teaches your brain that the compulsion was necessary. The cycle then gets stronger.
ERP interrupts that cycle. Gradually and at a pace you help set, you practice facing the situations, thoughts, or sensations that trigger your anxiety (exposure) while choosing not to perform the compulsion (response prevention). Over time, your brain learns that you can tolerate uncertainty and discomfort, and the obsessions lose their power.
Is It OCD? Common Signs
OCD looks different for everyone, and it is far more than a preference for neatness. You might recognize yourself in:
- Intrusive thoughts, images, or urges that feel disturbing, unwanted, or out of character
- Rituals you feel compelled to do, even when you know they don’t make logical sense
- Spending an hour or more a day on obsessions or compulsions
- Constant need for reassurance from friends, family, or the internet
- Avoiding places, people, or situations because of what they might trigger
- Distress, shame, or exhaustion about your thoughts and behaviors
Only a qualified clinician can diagnose OCD, and we’re happy to help you sort out what’s going on during an initial assessment.
What Can ERP Help With?
ERP is effective across the many ways OCD can show up. Our Asheville ERP therapists work with clients experiencing:

- Checking OCD (locks, appliances, safety, mistakes)

If you aren’t sure whether your experience fits, we’ll talk it through together during your consultation.
What Does ERP Therapy Look Like?
ERP is structured, collaborative, and goal-oriented. You are never surprised, never forced, and always part of the decision-making.
- Assessment: We learn about your symptoms, history, and goals, and how OCD affects your daily life.
- Education: You learn how the OCD cycle works and why compulsions and avoidance keep it going.
- Building your exposure hierarchy: Together we create a ranked list of triggers, from mildly to more distressing, so we can start where you feel ready.
- Practicing exposures: In session, and with practice between sessions, you gradually face triggers without performing compulsions.
- Response prevention skills: You build tools to sit with uncertainty and urges instead of acting on them.
- Relapse prevention: We plan for setbacks and help you maintain progress long after therapy ends.
Exposures are tailored to you, and they are always planned in advance and done with your consent. Your therapist works alongside you every step of the way.
What to Expect From Your First Sessions
Your first appointments are about understanding your experience and building trust. You won’t be asked to do anything that feels out of reach right away. Many clients feel relief just learning that their intrusive thoughts are common and that there is a proven path forward.
The length of treatment varies based on symptom severity and your goals. Research protocols often use weekly sessions over a few months, though some people benefit from shorter or longer courses. Your therapist will review your progress with you regularly and adjust the plan as needed.

Frequently Asked Questions
ERP stands for Exposure and Response Prevention. It is a type of CBT in which you gradually face feared thoughts or situations without performing compulsions, helping your brain learn that anxiety fades and that uncertainty is tolerable.
ERP is among the most well-studied and best-supported treatments for OCD, and many people experience meaningful reductions in symptoms. Results vary from person to person, and your therapist will track your progress with you.
ERP involves facing discomfort, so anxiety may rise at first, particularly during exposures. This is expected and temporary. Your therapist builds the plan gradually, explains each step, and never asks you to do something without your agreement.
You’ll share what you’re comfortable sharing, and we’ve heard it all. Intrusive thoughts, even upsetting ones, are a common feature of OCD and do not mean you want to act on them or that you are a bad person.
It depends on the severity and type of your symptoms and how much you practice between sessions. Your therapist will talk through a realistic timeline after your initial assessment.
Yes. Research supports telehealth delivery of ERP, and many clients do well with online sessions. This is available to clients located in North Carolina.
Yes. OCD often doesn’t respond well to reassurance, analyzing your thoughts, or talking through them at length, and these approaches can sometimes become part of the cycle. ERP targets the cycle itself through practice and behavior change.
For some people, medication prescribed by a psychiatrist or other medical provider is part of treatment. We can coordinate with your prescriber. Decisions about medication are always made with a medical professional.
Many insurance plans cover psychotherapy for OCD. [Insert your insurance policy here.] Contact us and we’ll help you understand your benefits.
